During Treatment: Maintaining Communication and Coordination

Throughout treatment, we maintain ongoing communication with referring providers to help ensure continuity of care and alignment across the treatment team. You have the opportunity to help shape the recovery journey rather than simply receiving updates or being on the sidelines. As your client progresses, we keep you apprised of treatment milestones, clinical observations, and overall progress while remaining mindful of appropriate privacy considerations.

Our treatment approach is built around the Recovery Story clinical framework, which offers structure for the treatment experience and provides a shared language and consistent parameters for clients and providers. Your continued involvement, then, is critical to treatment continuity and success. We will regularly communicate with you regarding your client’s progress and challenges. Throughout treatment, we also offer bridge sessions (either in-person or virtually) to create alignment between teams and reinforce treatment goals.

Discharge planning begins early in your client’s Recovery Story to ensure a smooth transition out of our program and back into their work with their outpatient team. To start, we will share all essential clinical information to support their reintegration. Additionally, your input will always be valued and considered as we determine the most appropriate next steps, whether your client is returning to your care or moving forward with another level of support.

Reconnecting Clients with Their Outpatient Team

When appropriate, we hope that our patients can return to care with their established providers. If they need additional help, we will offer recommendations to further support their recovery goals. During this vulnerable transition period, we strive to honor the relationships already in place while providing the additional resources needed to ensure continuity of care.

After Treatment: Supporting Long-Term Recovery

The Epilogue chapter of the Recovery Story is about preparing for what’s next. As clients transition out of treatment or into a lower level of care, our focus shifts toward helping them build confidence in their ability to navigate life’s challenges while staying connected to their recovery values and support systems.

To support this transition, each client receives a personalized roadmap for the next chapter of their journey. Through thoughtful discharge planning, aftercare recommendations, and practical recovery resources, we strive to create a safe, supportive bridge between treatment and everyday life.
